Prevent Mistake

You made a mistake. Look at the arguments, chat history, and other context to figure out what it was, then propose an update to instructions to prevent it from happening again.

1. Think hard about what the mistake was based on all available information

   • Use natural language to confirm your understanding with appropriate confidence
   • Examples: "I believe the mistake was...", "It seems like I...", "Was the issue that I...?"
   • Always phrase as a yes/no question to get clear confirmation

2. Once confirmed, identify which instruction file needs updating to prevent this

3. Propose a specific edit that would prevent this mistake from happening again

4. Apply the edit after user approval
